Directions to the RPM Health Farmington Office

Directions from Hartford:

1. Take I-84 West to Exit 39 (Route 4, Farmington) 2. Continue straight through the light at the end of the ramp onto Route 4 West and proceed to the next light at the intersection of Routes 4 and 10 3. At the intersection of Route 4 and Route 10, continue straight on Route 4 west; the office complex in which RPM Health is located is one mile ahead on the right; look for the entrance to the Forest Park Office Green, and turn right into the complex 4. Take your first left. RPM Health (Risk & Performance Management) is located at building # 3. We are on the 1st floor

Directions from Waterbury:

1. Take I-84 East to Exit 39 (left exit) 2. Follow steps 2 – 4 in the directions above

Directions from New Haven:

1. Take 91 North to Route 9 exit (towards New Britain) 2. Take Route 9 North to I-84 West (the road will split, stay left) 3. Take Exit 39 (Route 4, Farmington) off of I-84 West 4. Continue straight through the light at the end of the ramp onto Route 4 West and proceed to the next light at the intersection of Routes 4 and 10 5. At the intersection of Route 4 and Route 10, continue straight on Route 4 west; the office complex in which RPM Health is located is one mile ahead on the right; look for the entrance to the Forest Park Office Green, and turn right into the complex 6.
